Do you mean form in the Notes sense of the word, or just a document?

If you mean in a document then just write the field names and values using script (use the NotesItem methods or just extended syntax for NotesDocument).

If you mean that you wish to add fields to a form, i.e. design, then post back -- this is a bit more complex, and there are a few options you can try out.
